Make include guards standard-compliant

Description

Some include guards used a name pattern which is reserved for the implementation of a C++ compiler. This detail can be fixed by the deletion of leading underscores.I suggest to make the used prefixes also consistent.

This issue is a consequence from my bug report "reserved identifier violation".

Yes agreed - but this may take a while. I saw you raised the issue on the FastFlow site. When they sort it out we'll incorporate their changes.
